How to use the MediSAVE Card:
* Approximately 15 minutes before visiting the pharmacy, you must activate your card: Call 888-796-3320 (888-Rx-Med 20) or visit www.activatethecard.com/medisave.
* At the pharmacy, present your insurance card first and have the pharmacist process as “primary insurance.” Hand the pharmacist your MediSAVE Card to process as a “secondary adjudication.”
* If you are quoted a copay that is more than $35 for your 60 g tube of ZIANA Gel (including prior authorization), call 888-796-3320 (888-Rx-Med 20) for assistance before contacting your physician.
* Your card is good for savings on up to 3 prescription fills of ZIANA Gel (60 g tube only) within 6 months of activation or until the program expires (whichever comes first). See Eligibility Criteria for MediSAVE Card.
* If you have questions about the product prescribed to you, please talk to your healthcare professional.
